Position Summary:

Come join our team of healthcare professionals at River Crest Hospital. We are seeking a FT Assessment and Referral Coordinator to work evenings, Monday-Friday 3:00-11:00. This position offers an outstanding benefits package including a health insurance and 401K matching.

QUALITFICATIONS AT ENTRY:

Clinician responsible for communication with referral sources, discharge planning, case management and clinical therapies. Flexible hours, includes some weekends. Must possess organizational and time management skills; customer service oriented; skills in maintaining information as highly confidential, and is computer literate.
 
Requirements
KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge of clinical practices and procedures; knowledge of department and personnel policies
Skill in oral and written communication; skill in the operation of computer systems 
Ability to plan, organize and conduct discharge planning processes in a timely manner; ability to understand utilization of chart material.
This job requires understanding of and compliance with River Crest Hospital’s safety and loss control policies. 
 
EDUCATION AND /OR EXPERIENCE:
Minimum of a Bachelor’s degree from an accredited educational institution, psychology or sociology strongly preferred.   Master’s Degree preferred. Also prefer previous hospital experience in discharge planning.
PHYSICAL AND MENTAL REQUIREMENTS OF JOB:
Must be able to stoop, kneel, crouch, reach, stand for sustained period of time, walk, pull, lift objects from a lower to higher position or horizontally from position to position; express or exchange ideas orally and potentially loudly, accurately or quickly; perform repetitive motions of the wrist, hands, or fingers. 
Work is of medium demand both physically and mentally; walking, sitting or standing most of the time. Moving, lifting and turning of patients may be required. Must be able to assist in crisis intervention prevention/therapeutic holds on out of control patients. Must be able to perform CPR, run after a patient if needed, and lift and/or move 50 pounds. 
WORKING CONDITIONS:
Works in a well-lighted and air-conditioned environment with occasional periods of high stress.
