Rivendell Behavioral Health Services is a 77-bed acute care facility located in Benton, AR , has been providing quality health care to the residents of Arkansas since 1986. Rivendell provides and array of emotional and behavioral health services for adults, children and adolescents. Our multi-disciplinary treatment approach has created an environment for healing. Rivendell currently has an Assessment and Referral Representative  position available. The candidate selected will be responsible for the following:


Receives inquiry/referral calls, completes inquiry/referral form on each call, schedules assessment and completes follow-up on all inquiry and referral calls.
Assists in scheduling admissions of patients to hospital.
Records information that identifies referral source and patient.
Ensures all payor sources are verified. Completes financial packet when appropriate. Completes insurance pre-certification prior to patient's admission.
Completes admission process and distributes documents to the appropriate departments.
Record and screen clothing and valuables of patients upon admission.
Interview patient and family to gather information to assist licensed professional in determining appropriate level of care.
Obtain signature or appropriate consent and other admission forms from legally responsible person.
Assign patient and Account numbers.
Ensures communication of special treatment needs/concerns to appropriate departments through participation in continuity of care meetings or other means of communication.


Demonstrates the ability to gather pertinent information using the assessment tools provided.
Demonstrates the ability to be thorough when completing Part I and III
Demonstrates the ability to complete all blanks on the Part I and III
Schedules multi-family therapy appointments.
Demonstrates the ability to recognize signs and symptoms of severe anxiety and impending behavioral escalation
The social history is completed at the time of admission, where appropriate, as evidenced by documentation in the medical record.


Accurate information is communicated to all members of the team as observed by the supervisor.
Works cooperatively and positively with all treatment team members as observed by the supervisor.
Is flexible and creative with job requirements and expectation as observed by supervisor.

The requirements listed below are representative of the knowledge, skill, and/or ability required. Minimum of a Masters Degree in a Human Services field, preferably Social Work, from an accredited university. A minimum of two years psychiatric treatment experience is preferred along with experience in individual, group, and family therapy. Knowledge of regulatory requirements and Arkansas laws governing services to children, adolescent and adults preferred. A basic knowledge of educational, psychological and social service systems as related to special education and behavior treatment preferred.  RN license is preferred with minimum of Associate's Degree in Nursing.
